• Current residence
• Social Security Number
• Date of Birth
• Current Place of Employment
• Salary
• Monthly bills (excluding utilities), and their approximate balance

If you’re going to come in to the office, please bring:

• A photo ID
• A paycheck stub
• Your car title (for security on large loans)
